UNDERSTANDING BIPOLAR DISORDER

Bipolar disorder treatment in Olympia, WA — expert care for a complex condition

If you or someone you love in Olympia, WA or Thurston County is living with dramatic mood swings — periods of intense energy, reduced need for sleep, and elevated mood followed by deep lows that make it hard to function — bipolar disorder may be at the root. It is one of the most misunderstood and misdiagnosed psychiatric conditions, and without the right diagnosis and treatment plan, it tends to get harder to manage over time.

At Healthy Minds Psychiatry, we understand that bipolar disorder requires a different level of care than many other mental health conditions. It is not something to manage casually. Dr. Kler brings the clinical expertise, the time, and the long-term commitment to build a treatment plan that actually stabilizes your mood — so you can get your life back.

"Bipolar disorder is not a personality trait, a dramatic temperament, or a reason to write someone off. It is a serious and treatable medical condition — and with the right care, people with bipolar disorder live full, meaningful, and stable lives."

WHAT IS BIPOLAR DISORDER?

More than mood swings — a serious condition that affects the whole brain

Bipolar disorder is a chronic mental health condition characterized by significant shifts in mood, energy, activity levels, and the ability to carry out daily tasks. These shifts go far beyond typical emotional ups and downs — they are distinct episodes that can last days, weeks, or months and significantly disrupt work, relationships, and quality of life.

According to the National Institute of Mental Health, bipolar disorder affects approximately 4.4% of adults in the United States at some point in their lives. It is often misdiagnosed as depression, anxiety, or borderline personality disorder — sometimes for years — because the hypomanic and manic episodes are not always recognized as part of the same condition.

TYPES OF BIPOLAR DISORDER

Understanding the different presentations of bipolar disorder

Bipolar disorder is not one-size-fits-all. There are several distinct types, and accurate diagnosis is essential because treatment approaches differ significantly between them:

TypeWhat It Involves
Bipolar IDefined by manic episodes lasting at least 7 days, or severe enough to require hospitalization. Depressive episodes typically also occur. Bipolar I is the most severe form and requires careful, ongoing medication management.
Bipolar IICharacterized by a pattern of depressive episodes and hypomanic episodes (less severe than full mania, without the functional impairment). Bipolar II is frequently misdiagnosed as depression because hypomania can feel productive or even positive.
Cyclothymic DisorderA milder, chronic form involving periods of hypomanic and depressive symptoms that don't meet the full criteria for either type. Symptoms persist for at least two years and still significantly affect daily functioning.
Other Specified & Unspecified BipolarBipolar-spectrum presentations that don't fit neatly into the above categories but still cause significant distress and disruption. Accurate clinical evaluation is key to identifying these presentations.

RECOGNIZING THE SIGNS

Symptoms of manic, hypomanic, and depressive episodes

Bipolar disorder involves distinct episode types — and recognizing both sides of the cycle is critical to getting the right diagnosis. Many people seek help only during depressive episodes and don’t report the high periods because they don’t feel like a problem at the time:

During Manic / Hypomanic EpisodesDuring Depressive Episodes
• Unusually high, expansive, or irritable mood• Persistent sadness, emptiness, or hopelessness
• Decreased need for sleep without feeling tired• Fatigue and loss of energy most of the day
• Racing thoughts and rapid, pressured speech• Loss of interest in activities once enjoyed
• Inflated self-esteem or grandiosity• Difficulty concentrating, remembering, or deciding
• Increased goal-directed activity or agitation• Changes in sleep — too much or too little
• Impulsive decisions — spending, risky behavior, substances• Thoughts of death or self-harm
If you or someone you know is experiencing thoughts of self-harm, suicide, or a psychiatric emergency, please call or text 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room immediately. If you are a current patient experiencing a crisis, please contact our office or seek emergency care.

OUR TREATMENT APPROACH

How Healthy Minds Psychiatry treats bipolar disorder

Bipolar disorder requires a long-term, specialist-level treatment relationship — not a one-time appointment. At Healthy Minds Psychiatry, we bring the clinical depth and continuity of care that this condition genuinely requires:

1

Comprehensive psychiatric evaluation and accurate diagnosis

Accurate diagnosis is the foundation of everything else. Dr. Kler conducts a thorough clinical interview covering your full mood history — including episodes that may not have seemed like symptoms at the time — your medical history, family history, and any previous diagnoses or treatments. We take the time to get this right because the wrong diagnosis leads to the wrong treatment.

2

Mood stabilizer and medication management

Bipolar disorder typically requires mood-stabilizing medications — lithium, anticonvulsants, atypical antipsychotics, or a combination — that are carefully selected, titrated, and monitored over time. Dr. Kler manages your medication with regular follow-up, lab monitoring where required, and careful attention to side effects and therapeutic response.

3

Coordinated physical health support through our team of ARNPs

Some bipolar medications require routine lab work and cardiovascular monitoring. Our team of ARNPs handles the primary care side of your health — including any physical health conditions, metabolic monitoring, and medication interactions — so your mental and physical health are managed together, not in silos.

4

Psychoeducation and relapse prevention planning

Understanding your own patterns — triggers, early warning signs, sleep disruption cues — is one of the most powerful tools in long-term bipolar management. We work with you to build awareness of your individual cycle and develop a clear plan for what to do when symptoms begin to shift.

5

Referrals for therapy when appropriate

Medication is essential in bipolar disorder — but it works best alongside evidence-based therapy such as cognitive behavioral therapy (CBT) or interpersonal and social rhythm therapy (IPSRT). When therapy would strengthen your treatment, we provide referrals to trusted therapists in the Olympia area.

6

Ongoing long-term monitoring and care

Bipolar disorder is a lifelong condition that changes over time. We schedule regular follow-up appointments to monitor mood stability, review medications, track any emerging symptoms, and adjust your plan as your circumstances evolve — for the long haul.

FREQUENTLY ASKED QUESTIONS

FAQs about bipolar disorder treatment at Healthy Minds Psychiatry

How is bipolar disorder different from depression?

Depression is characterized by persistent low mood without significant high periods. Bipolar disorder involves both depressive episodes and manic or hypomanic episodes — periods of elevated or irritable mood, reduced need for sleep, and increased energy. Many people with bipolar disorder are initially diagnosed with depression because they seek help during a low, not a high. A thorough psychiatric evaluation is essential to tell the difference — and it matters, because the treatments are different.

Do I need a referral to be evaluated for bipolar disorder?

No referral is required. You can contact Healthy Minds Psychiatry directly to schedule a psychiatric evaluation with Dr. Kler — either in person at our Olympia clinic or via secure telehealth anywhere in Washington State.

Will I have to be on medication for bipolar disorder forever?

Bipolar disorder is a lifelong condition for most people, and ongoing medication management is typically recommended to maintain stability and prevent future episodes. However, your treatment plan is always built collaboratively — we discuss the evidence, your goals, and your preferences at every step. Medication decisions are never made without your full understanding and input.

Can you treat bipolar disorder alongside anxiety, ADHD, or PTSD?

Yes. Co-occurring conditions are common in bipolar disorder. Dr. Kler evaluates and treats co-occurring anxiety, ADHD, PTSD, and other psychiatric conditions together — so your care is integrated and coordinated rather than fragmented across multiple providers.

What if I've been diagnosed with depression before but I think it might be bipolar?

This is more common than you might expect. Bipolar II in particular is frequently misdiagnosed as depression for years because the hypomanic episodes can feel like a return to normal rather than a symptom. If you suspect your diagnosis may be incomplete, Dr. Kler will conduct a full evaluation and work through your history carefully — without rushing to conclusions in either direction.
